Help Me Grow Home Visitor
QUALIFICATIONS: At least an Associate's Degree in human services or a field closely related to early childhood or a Bachelor’s Degree with a minimum of one-year experience working with pregnant women, infants or toddlers. Bachelor’s Degree Preferred. Must obtain within the first year of employment and maintain a Help Me Grow Home Visitor Credential with the State of Ohio. Undergo a criminal background check with the Ohio bureau of criminal identification and investigation and a background check with the Ohio central registry on child abuse and neglect. A valid Ohio driver’s license with own vehicle and ability to maintain a driving record that meets the insurability requirements of Miami County Public Health’s insurance provider.
DUTIES: Provides family support, information and education, developmental instruction and follows program goals. Provides regular home visits to enrolled Help Me Grow families with children from prenatal up to three years. Plans for each visit and teaches effective parenting and child development skills. Provides parents with information on disciplining, behavior management, household management, communication and relationship enhancement when needed. Utilizes curriculum and implements evidence-based model when providing services to families. Conducts developmental screenings. Facilitates and participates in the development, implementation, review, and monitoring of the family plan. Works with the family to ensure they have medical/home health services. Coordinates and collaborates with other service providers. Provides referrals to other service providers as needed. Participates in miscellaneous program activities (e.g. assists in program development, child find outreach and intake activities). Completes documentation, which includes the client’s paper chart, data entry into OCHIDS, reports and time and mileage sheets. Assures that parents have been informed of their rights. Maintains and increases knowledge and skills through attendance at meetings, conferences, trainings, and other continuing education opportunities. Participates in emergency response and accreditation related activities by supporting and engaging staff in the accreditation process. Attends staff meetings and serves on temporary committees/teams, including Continuous Quality Improvement and Strategic Planning.
